Patients, family members, and caregivers are welcome to initiate a referral. We’ll explain the referral and visit process, answer questions about getting started, and work with you to coordinate the next steps for mobile wound management.

For providers

We welcome referrals from physicians, nurse practitioners, physician assistants, home health agencies, case managers, and other members of the care team. Send facesheet, supporting records, labs and diagnostic results through our secure referral process or by fax to 844-40-WOUND. We coordinate care and communicate relevant findings, recommendations, and follow-up needs with the referring and treating team.

For facilities

We partner with assisted living communities, group homes, skilled nursing facilities, and long-term care teams to provide on-site wound management for residents. Send facesheet, supporting records, labs and diagnostic results through our secure referral process or by fax to 844-40-WOUND. Partnerships may include scheduled wound rounds, coordinated clinical follow-up, staff education, and prevention support tailored to the needs of your residents and care team.
